Player Behaviour Agreement*

I have read, understand, acknowledge and agree to participation in the State Emerging Players Program on the terms and conditions outlined in the Participation Agreement and agree to be bound by the Codes of Conduct applicable to Players and all associated components applicable to players set out in Appendix B (together, ‘Cricket Victoria Emerging Player Program Behaviour Policy’). In particular, I acknowledge and agree and if I breach the Cricket Victoria Emerging Player Program Behaviour Policy, I accept all consequences, including any associated costs.

Parent/Guardian Consent*

I am the parent or guardian of the Player. I have read, understand, acknowledge and agree to and child’s participation in the State Emerging Players Program on the terms and conditions outlined in the Participation Agreement (including but not limited to the Cricket Victoria Emerging Player Program Behaviour Policy) and accept all consequences of my child’s breach of the Cricket Victoria Emerging Player Program Behaviour Policy, including any associated costs. I understand that my contact details may also be passed on to Victorian Premier Cricket clubs. Further, I agree to be bound by the Codes of Conduct applicable to Parents/Guardians in Appendix B and agree to the Parent/Guardian Declaration and Consent.
